Description
Geriatric Outreach Assessment Team is a coordinated network of specialized assessment, treatment and rehabilitation services for elderly persons whose well being, independence, or functional ability is threatened by multiple health or social problems, hosted by the Ottawa Hospital and a product of the combined efforts of many hospital and community partners.
