The East Ottawa Community Family Health Team Memory Clinic is based on the memory clinic model for primary health care, outlined by Dr. Lee and supported by the ARTIC initiative. The organisation provides evaluation from a multidisciplinary team composed of a nurse, a nurse practitioner, a social worker, a pharmacist and the family physician and provides treatment options and resources. Provides help to people whose memory is interfering with their everyday life and requires a physician or nurse practitioner referral.
