Description
The Ottawa Hospital Geriatric Medicine Clinics is committed to working with family physicians and community service agencies to provide accessible healthcare to patients in need. The Geriatric Day Hospital provides a comprehensive multidisciplinary assessment for patients who are experiencing a change in function, memory, mood, or have complex medical issues. Short-term treatment, counselling, and education are available to patients and their caregivers to facilitate community support and long-term care planning. The first meeting is 3-4 hours with a light lunch and coffee provided. They also offer an outreach program that provides a comprehensive in-home assessment in order to identify appropriate services and support for seniors with complex health problems.
